*Job Overview::*
Join our Primary Care Team and coordinate the timely care for patients!
You will work with a variety of patient populations and those with chronic disease(s) who receive care in clinical and home-based setting. Track and maintain case management/coordination to ensure timely referrals and scheduling. Monitor patient progress and track outcomes.

*Education:*
* Associate’s degree or 2 years of college in social work, social services, healthcare administration, business administration or related field *OR 2 years of work experience in any industry.*
* A Bachelor’s degree in a relevant field may be exchanged for all required experience.

*Experience REQUIRED:*
* 2 years of healthcare, behavioral health, dental, or relevant administrative experience required.

Job Type: Full-time

Pay: $26.06 - $36.08 per hour

Expected hours: No less than 40 per week
