Job Details

• Interventional scrub for multimodality interventional suite including Cardiac Cath, EP, IR, Vascular, Neuro IR.
• Preferred to have documentation experience with Merge Hemo.

Job Requirements

• Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals.
• Completion of Ascension Specialty Exam prior to first assignment and updated every 3 years thereafter in specialty with minimum 80% passing score.
• Completion of Ascension Core Competencies including HIPAA, Joint Commission -NPSG, OSHA Topics within a year of start and updated yearly with minimum 80% score.
• Documentation of vaccination series completion or declination for Hepatitis B, Influenza Vaccine, MMR, Varicella, and Tdap.
• Fit Test required for all travelers, preferred prior to start, can be completed onsite.
• Positive PPD with negative chest X-Ray and annual TB questionnaire if applicable.
• Statement of good health or completed physical exam prior to first assignment, updated annually thereafter.
• Driver's License/State ID must be current and match address on file.
• Completion of Ascension Skills Checklist within one year of start; updated annually for specialty.
• Ascension Criminal Background Check including Social Security Trace, Employment Verification, OIG Search and National Sex Offender within 30 days of first assignment, updated annually thereafter.
• Ascension EPLS run within 90 days of start, rerun if break in service >90 days.
• Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually thereafter.
• Verification of education relevant to clinical work within 90 days of start.
• Third-party employment verification for past seven years, updated annually thereafter.

Schedule Information

• 7 days on call including 1 weekend in 4 week period.

Additional Details

• Ascension Resume required for submission.
• Ascension Badge Photo optional: forward facing headshot with white or blue background, no alterations or filters.
• Ascension Exit Survey optional: to be completed within 72 hours after assignment completion.
• Signed Assignment Agreement required for onboarding.
• Ascension Medical Imaging Contract Worker Onboarding Education Attestation required for onboarding.
• Ascension Medical Imaging Contract Worker Onboarding form completed onsite.
• Ascension Profile (Resume) required for submission.
• Ascension 2 Professional References required for submission.
• Ascension Confirmation of Assignment required for onboarding.
• Ascension Employment Verification required for onboarding.
• Hiring Manager Evaluation optional post-assignment evaluation.
• Ascension Name Change Document optional.
