Job Description

The Social Worker Senior position is located with the Metro Social Services and will lead and develop individual and group sessions as well as clinical case management that address the clinical issues of the customers that we serve. Incumbent is also responsible for establishing the policies, procedures, documents, and guidelines that are necessary to successfully coordinate with the appropriate partners. This position works with all populations, except for minors including, homeless individuals and families, non-homeless individuals and families, ex-felons, domestic violence, elderly, veterans and sex offenders etc. Metropolitan Social Services (MSS) is in the business of helping Nashvillians who are experiencing hardship. Services include counseling, information and referral, case management and other assistance. Determines the needs and eligibility of persons seeking assistance. Assesses eligibility according to program policies, procedures, and guidelines. Provides information and referral services to customers and the public. Provides the customer with all the information to enable them to understand the programs services, and the information needed to understand the continuation of services. May make home visits to customers or to other agencies or locations in the community. Maintains accurate and professional case records and contacts. Completes all needed documentation and forms completely, accurately and in a timely manner. Responsible for entering information and data into the case management system. May be responsible for creating reports for program data. Responsible for maintaining a positive and healthy working relationship with the Coordinated Entry partners and ensuring that the Coordinated Entry customers receive the appropriate services via our referral and case management services within the Coordinated Entry guidelines. Responsible for providing case management for vulnerable populations. Responsible for coordinating and leading various groups around trauma informed care, two-gen, ACE’s, life skills management and de-escalation. Works with staff that develop group sessions and monitors the ongoing data, relevance, and efficacy of the groups. Works closely with and/or within other units as it pertains to the customers. Responsible for working and taking lead on agency projects and assignments as needed. Responsible for any special projects, agency events and community events as needed. Assists with establishing policies, procedures, documents, and guidelines for programs. Works closely with Program Management and the Executive Director, and may be asked to represent the agency on behalf of management. Develops, markets, maintains and nurtures community partnerships. Attends meetings and trainings as needed. Will work in the community and do outreach when appropriate. Assumes other duties as needed and/or assigned.

EDUCATION AND EXPERIENCE

Master’s Degree in Social Work from an accredited college or university and three (3) years experience in social work, program management and/or program development. No substitution.

LICENSES REQUIRED

Valid Driver License may be required for some positions in this classification. Candidates with accreditations earned in a foreign institute are encouraged to apply. Note: Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government. Experience supervising staff. Experience working in Microsoft Office software (i.e. Word, Excel, and Access). Licensed Clinical Social Worker (LCSW).
